STANLEY R. FRIESEN is Professor of Surgery and the History and Philosophy of Medicine at the University of Kansas. IVAN D.A. JOHNSTON is Professor of Surgery at the University of Newcastle-upon-Tyne. RONALD A. SELLWOOD is Emeritus Professor of Surgery at the University of Manchester.
